to me it's a very well done murder ballad - one of Tom's many. This is 2 part narrative by the killer, and Alice the victim.
Some clues..
1. Your hair is like meadow grass on the tide ( visions as he dumps the body)
2.And so a secret kiss madness with the bliss (temporary insanity)
3. Alice's ghost says..
Arithmetic, arithmetock
Turn the hands back on the clock
How does the ocean rock the boat?
How did the razor find my throat?
The only strings that hold me here
Are tangled up around the pier

The Ghost of Alice could be just his insanity, voices in his head

The killer is out on the frozen lake literally skating the word Alice on the ice.

Theres a lot of other stuff woven in here, but this is I believe the essence of the song..

To me..it's definitely a story about a guy who who gets shot in Maxwell St, Belzoni Mississipee for messin' with Tullulah (probably Belzoni's the saw mill owners's wife - maybe the town was named after him) He reflects on his travels and his life as he lays dying in Maxwell St , full of hollow after being shot by Burnt face Jake . Looks up sees his pony running away confused - life passes before his eyes as he lays dying
