To me, the song reminds me a lot of PTSD. Katniss is haunted by the memories of what happened in the Hunger Games and perhaps the aftermath of her actions outside those games.
With the line "I'm finally at peace, but it feels wrong", it's like she's come to terms with what's happened, but she feels guilt for surviving when so many others died. Similarly, in "There's nothing I'd take back, but it's hard to say there's nothing I regret" she knows her actions were necessary at the time but she still wishes there was some other way to work things out.
In "my hands and feet are weaker than before", these feelings are manifesting physically, perhaps affecting her will to continue on as normal. I agree with the other poster that the other person mentioned in the song is Peeta. (The song could be from Peeta's perspective for some parts since they experienced the same thing. I'm assuming the song is mainly from Katniss's POV since that's how the movie plays out.) At the end of that verse, darkness becomes her since that's the best way to escape from it all.
Finally, with repeating "I'm already there", she's telling these nightmares and later a thousand silhouettes (fallen tributes and other victims) that it's not necessary for them to haunt her anymore. She's already there in her emotions and there's no way for her to forget regardless.
That's just my guess. Either way I think this song is amazing and I've been in love with it since I heard it at the theater. :)
