to me this has always meant that he wants it straight. don't beat around the bush, don't dress up your sentences with meaningless filler words to make your story sound easier to buy; don't make things more dramatic than they should be.

"don't call me by my full name, all this is temporary"
this makes me feel like maybe someone is again taking things too seriously for him. that person has no right to call him by his full name when he doesn't really care for them as much as he says he does. it's temporary for that reason. because of this, "it feels much better to know, that you won't feel a thing" feels like sarcasm to me.

did you ever wish you were somebody else?"
maybe he's gotten himself involved with this other person and is realizing that he doesn't want to be anymore...but he can't explain this to them, he can't even explain it to himself. so, he asks this significant other, "did you ever wish you were somebody else?" in an effort to have them attempt to relate.

"accomplishments are transient"
"just lasting this long, I feel relieved"
even if they got through a shitty situation in the past...now, it doesn't matter; their former accomplishments have become minorities in the grand scheme of problems in their relationship. he feels like it's pretty shocking that he's gotten this far with this significant other. "let repetition save me" -- he's fallen into a pattern, they're stuck in a rut, they won't ever get out.

to me, the song is called "the difference between medicine and poison is in the dose" because this significant other of his is just that: at first, they made him happy, fixed what was temporarily wrong...now, they're poison; they're both simultaneously ruining the relationship. anthony (or whomever he's singing from the point-of-view of) can see it falling apart, but he won't own up to it; he's comfortable and doesn't want to change anything that he's comfortable with. the significant other does not seem to realize his feelings, but he/she does notice that his mood has changed - the sarcasm, the carelessness.

to cover my ass: this is just my interpretation. I'm aware that anthony green is married and has a child; I was just attempting to create an ambiguity with the "significant other."
