I think the "Yellow" in this song is about a symptom of a disease known as jaundice.

Jaundice can be caused by several diseases like hepatic dysfunction and disease, inefficient thermoregulation such as with premature infants and several blood factors. In every case, when red blood cells in the body are destroyed in increased or normal amounts, the end product is called bilirubin which in increased amounts is toxic to the body - and is also colored yellow. Bilirubin is normally excreted in the body by passing through the liver. When the liver is diseased or dysfunctional, it fails to excrete the bilirubin. The retention of bilirubin, called hyperbilirubinemia causes signs of yellow skin as well as eye coloration - thus, the word “Yellow.”

I think the song is about someone he loves who is sick or dying. That person has loved and taken care of him at one point or most of his life. I’d say it was his mother.

He feels it’s his to give back the love his mom had given him. She is sick or probably dying and he took the chance to show her his love. He visits her in the hospital and writes her a song.

And even during the moment that her body is wasting away, he sees beauty in her. And in that moment, he knows he’ll sacrifice everything for her including all his blood. Blood transfusion is a common intervention at extreme exacerbations of liver dysfunction.
