I think this song is brilliant! There is so much beauty in the composition of this song it almost doesn't matter what its about. Only recently did I put this idea together, so bare with me. I think this song is about his thumb. He uses his thumb for strumming and the piccato style playing he does with his violin. He complained once at a show I saw in 2009 about how his thumb is always calloused and a sore because he plays so often and he gets hung up on his guitar strings because of it. He may have gotten an infection on his thumb and went to the hospital to discover he had parasites inside his thumb, which would be very intense, especially if it was a rare parasite.
