This is Steve's official comment on a later rework/compilation:
"Another Genesis branch, this time inspired by a dream I had of Genesis performing in a surrealistic version of New Orleans."
By 'Genesis branch' he means a song he wrote for Genesis which was rejected and which he reworked later for his solo albums.
It does actually sound quite Genesis-y, at least the instrumental breaks. I love the first verse, especially the first few lines - "The Mississippi sighs" is wonderful.
Incidentally I don't think there is a 'Camino Royale" in New Orleans anyway. It's a strange name, being a masculine Spanish noun for 'road' followed by a feminine French adjective for 'royal'. Perhaps Steve was thinking of this? https://en.wikipedia.org/wiki/El_Camino_Real_de_los_Tejas_National_Historic_Trail which at least approaches New Orleans. I suspect the phrase was also inspired by 'Casino Royale'.

This is the explanation I have developed in the last few days as I have got to know this amazing song.

It was a dream of Chibi's. Maybe she edited what she dreamt to better fit a song, but the constant "I think" may actually be her vague recollection of what she dreamt rather than the narrator only vaguely remembering because she was on drugs or out of her mind. Maybe it connects with her being bullied at school, indeed a revenge fantasy but not a waking one but a dreamland one where the subconscious is allowed to entertain socially unacceptable actions. The timeset could be high school, but the fact that it seems to be her male companion's birthday while she has a room in the same building or at least nearby makes me think it may be student accommodation where a group of young people live together.

I think there are only two companions, not three - the female narrator and her male 'friend', whose birthday it is. I think he is real within the story, even tho some of the way it's told does make it sound like he could be an imaginary figure in her head. He could be more than a 'friend' as the chasing her to her room and the last verse do sound like sex. It is possible he kills her and the chasing is hostile and the video/smile bit is him being particularly sick about that but my first thought was the chasing was fun and mutually wanted, and him telling her "Don't forget the video" wouldn't make sense if she was not in on what they were doing - is he not asking her to basically switch a video camera on as they're about to have sex?

The murder victims seem to be birthday party guests, but I wonder if they're not necessarily human. They could be animals, or dolls. You have to ask yourself how it is possible for two people to murder an entire room of guests without resistance. If the narrator's dress is stained red with blood, that's a lot of stabbing/slashing and spurting. However, if it's a dream some of the unrealism becomes unimportant; unrealistic things can just happen in dreams. You turn up with your friend/lover and it's possible to knife a whole roomful of people without being hurt or fought off.

Same point about the wishing them happy birthday and kissing them goodnight. What sense does it make that they wish all these now dead guests happy birthday, when it's the boy's birthday? Answer: it's a dream, it doesn't have to make sense. What, you went round them all and kissed them on their dead lips and said "Goodnight!"? It's a dream, you can do that sort of thing.

A couple of lines puzzle me a bit. "I hear footsteps" and "Two of them are sisters". The latter makes me wonder if they are the boy's birthday party guests at all. Maybe he is the birthday boy, but they just invaded or gatecrashed somewhere else and killed who was there, and he noticed the family relationship between two victims and pointed it out.

That's it - could all be wrong but that's my best shot, and what I prefer to believe. It's a wonderful song, really worrying lyrics but fabulous tune and Chibi is so hypnotic and insidious with her singing, I too have to keep replaying it to get another hit of the vibe it gives you. This is the song that gave them their name, as they used to be called Imajica and this was called the Birthday Massacre, but they found there was another band with the name Imagica so they changed their name to the Birthday Massacre and the name of this song to Happy Birthday.
