To me, it seems pretty clear that this is about the same thing as "Buildings," which was about how Regina's father took care of her depressed, alcoholic mother out of "true love" and his hope that she would get better one day.

She'll break her own heart"

^He knows that spending his entire life cleaning up after her and carrying her burdens will break his heart. The song suggest he should "let go of her hand" and let her pull herself back together to spare his own heart.

"She's been skipping days spilling her drinks in the sink"

^She's skipping work, drinking.

"And you know she never coming home never coming home again"

^Deep inside, he knows she will never fully recover from her depression or alcoholism.

"But when, when, when she open her eyes, eyes, eyes beyond the chipping paint through the windowpane

^It gives him hope when she "opens her eyes" and pulls herself together for a time, but he soon finds these periods of recovery are "lies" and will never be permanent.

"Her patron saint broken and lame and absolutely insane for learning that true love exists"

^He is her "patron saint." He sacrificed his life for her benefit. He is "insane" for going to such lengths, but he does so because he knows that she is his true love.

"You'll be to blame for playing this game and learning that true love exists"

^He can only blame himself for sacrificing himself to the "game" of true love.

"The pain, the pain, the pain of knowing that true love exists"

^His love and his awareness of it has caused him lifelong pain.
