It is possible (indeed, likely) to be depressed before you commit suicide. The song may or may not be written with Cobain in mind but the timeline does not exclude the possibility. Besides the song sounds to me to be written for someone who is not yet dead. How can you "hold on" if you've already let go.

I can't dissect every single lyric but the general feeling I get from this song is one of a husband feeling trapped.
I don't hear friends talking to him, for me the narrator is either himself, the wife or a mixture of the two.
The husband definitely feels like he could have done more with his life if he hadn't had kids. He defines himself as a father but he doesn't seem happy about it.
But he also seems to fear what would happen if he did lose his family. The whole second verse has him exploring what it would be like to be alone (or possibly he is alone by this time). All the imagery here reminds my of the Cobb/Mal story in Inception.

The thing that sticks out for me most is the last couplet. I can't decide who is talking here but it's certainly spouse to spouse - "Dear". It conjures up images of a party where one says to the other in front of friends "Dear we better get a drink in you before you start to bore us." This could be seen as a playful jab, a reminder between lovers that not everyone is as interested in "whatever" as you are. But given the tone of the song that precedes it it takes on a dark, passive aggressive tone. And the way Matt screams it at the end (fantastic live btw) really makes me think the comment was not intended nicely. Whoever says it, the sentiment shows the marriage is doomed.
