I think, like some others, this song is about accepting death. First of all, e said he was a mountaineer, so that could mean there was an accident or something?
The first clue is "I fall asleep in hospital parking lots".

The next line is
"And Awake in your mouth".
I think at this point, the patient is entering a dreamlike coma state. They are mixing things up. The line
"I am a cold seahorse" Seems to relate to that of a dead person. They would feel cold.
When it says "I am an old lighthouse throwing beams of bright lights. Red in the morning Blue in the evening."
Normal Lighthouses do not have red and blue light. In his mind, he is imagining the lights on an ambulance as lights in a lighthouse. Now it replays some lines. After the next "I fall asleep in hospital parking lots" The beat/drums that were there throughout the song stop. I think the drum beat is kind of like the persons heartbeat. This signifies the death of the person. Then,

Take me above your light
Carry me through the night
hold me secure in flight
sing me to sleep at night.

I think this is talking about Adam's relationship with god. He is accepting death because he knows that he will be safe with god, and to hold him secure so that he is always with god.

This song, and many others of Adam's can be interpreted so many different ways. That's why I love them.
When I hear this song, I can think of multiple meanings.

1:
He's not actually in a different "World". He's just saying that he's different than everyone else, and sees everything differently. If he could choose to see things like everyone else would, he wouldn't.

2: It could be that he feels like he's an another world when he's with the person that he's talking about in the song. He's saying that reality was nice, but now that he's seen this he would never want to go back.
