Almost. He says himself "I might not want you back...". He knows he and this girl aren't right for each other, and that they, in the end, are better off seeing other people, however, it's still too painful for him to think of her being with anyone else, simply because they've been together for so long. This song's about jealousy much more than it is about longing. |

He's a modern leper in the sense that he's not a literal leper; he doesn't have leprosy. He is, however, still a kind of leper in his own right. He feels that his actions are causing people to stay away from him, resulting in feelings of isolation.
In that first set of lines you mention, he's being ironic. Obviously, simply stating that they're both alive doesn't make them a perfect pair, he just wants to be with this girl and will accept any justification. Notice he doesn't say that it does make them perfect, he poses it as a question. It's rhetorical, probably because he doesn't want to have it answered, fearful of what that answer might be.
This song is cathartic in a lot of ways, it's about him owning up to all the self-defeating shit he's been pulling for too long. The cutting off of his foot to spite his leg is an illustration of the extent of his self-destructive behavior. Cutting off his foot would spite his leg because the leg needs the foot to walk on, the figurative significance here being that both the leg and the foot are parts of him, and that in his self-loathing, all he does is spite himself, in the process making himself even more worthy of loathing. He's cutting out the good stuff (read: his relationship with this girl he loves) because he can't help it, even though he knows it's wrong to do, he can't stop himself from pushing people away, and this is what makes him a leper. The modern refers to, i would imagine, all young people in relationships, and them staying scared is a reference to the difficulty and emotional strain that relationships tend to cause. |

It's more than a bit sad, I think. I agree with BDanahy, he feels the relationship dying, and he just wants things to go back to the way they were, but at the same time, he knows it's impossible. They're both relying on memories, the only reason they're still together is because they made each other happy at some point, and that's what he means when he says "old fashioned". He gets more and more desperate as the song goes on, he spurns electricity, as he pleads to get old fashioned |

That's what makes the song so heartbreaking though, it doesn't help. He keeps saying "I need company, I need human heat" and he's still saying it by the end of the song; he never finds it. On top of that, he realizes that she is just like him and that she's looking for the same thing. "The Twist" is that even though they're having sex, that sex doesn't generate the kind of heat either of them are looking for, they're more or less just ships passing in the night. This is consistent with "Keep Yourself Warm" when he says "it takes more than fucking someone to keep yourself warm". |
