I have seen the comment by the guy who talked to Matt Berninger, but here are my original thoughts anyway:

I first thought of this song as a tale of a man who lost one of his best friends to a religious group / cult. I have had two acquaintances who I/we have "lost" to these groups. One became a scientologist the other became a part of a very closed off group of Jehovas Whitnesses. They both had to / chose to cut off all of the people in their previous lives, family, friends ect... Their parents can't even get in touch with them.
Their family and close friends (like the protagonist of this song) believe that they want to leave the groups but cannot get away from them. And here is where the line "Fake a heart attack, you gotta get back to me" fits in perfectly.
Also the whole thing about listening to a very persuasive man who got them interested in the groups in the first place, matches the "balloon-man"/ air head from the lyrics.
