I think this song is basically about abusing drugs. He's the doctor, and he's the patient. He controls what "medications" he takes and the dosage. They don't even have to be prescriptions like the song's title but it is very possible that they are, or he's taking prescriptions and illegal substances. The one making the decision between screwing all his fun and filling his prescription could simply be a drug dealer. To me, the verse: "If ya love me, like I love me Everybody will be sorry" ...seems to be related to everyone's views of him. The only people who will be sorry are the ones who love him like he loves himself. It sounds like a reference to how people stop caring about drug addicts over time (usually because of being screwed over by said addict). He's also in denial to begin with, where he's saying he doesn't need anything before the show, and then he becomes more addicted and dependent and needs something before the show to feel good again.
