You'll always be tied to what you've done and at the end of the day if you feel like you're not a good person, then the only resolution is to attempt to change your nature. Yet, you're trapped forever in your own mind with the memories of what you've done in your life. If you have corrupted and harmed other human beings, then it's difficult to feel like you really deserve forgiveness. If you can't look at yourself and say that you've lived your life the right way or treated people well, then how will you ever feel like you're deserving of the good that life has to offer?

No. That is the setting for the song and a vehicle for the meaning of the song. The images that make you think of a homeless man are representative of a very specific point in one's life, an emotional revelation so to speak, and are perfect for the emotions Hetfield is attempting to evoke.
