Face value, it seems to be about strangling someone to death, but there is a distinct lack of hostility in the tone of the chorus. In the whole song, actually, it's more calm than aggressive, so maybe it's a mercy killing? Or maybe just a completely apathetic killer.

On another level, some of the lines, "You look at my face, but don't see me. The nails deep in the flesh. I whisper in your ear, but you don't hear me. Your scream in my skin. I hold you tight to me, but I don't feel you. My sweat in your hair. Finally you know me." Those lines hint pretty hard to sex and orgasm. Add in the rest of the lines, and I guess it's rough sex, but again, the delivery of the song is calm and strikes me as a little loving. Add in the concept of Le Petit Mort (the little death) http://en.wikipedia.org/wiki/La_petite_mort and you've got a very interesting perspective on sex, I think.
