I think that everyone's "metaphor for life" argument is reasonable, but i disagree, and it's kind of over semantics.
The song is all future tense, things WILL happen, however the movie metaphor is present tense, "you still DON'T like to leave before the end of the movie". If it said "you still WON'T like to leave before the end of the movie", the life metaphor would make sense because it's suggesting the possibility of suicide, which would only come after all the bad stuff happens.
Because "don't" is present tense, it's separate to the bad things. To me it means that all this bad stuff is going to happen in life, but people are still dumb enough to waste the time that they do have watching credits, something so pointless, instead of doing something that will make your life better.

The first verse is about disproving the stereotype of people who are HIV positive. Someone he loves, probably a girlfriend, is *not* a drug addict or a prostitute: "Not bloody cotton swabs and lies
Stolen checks and empty eyes". Instead she's all of the beautiful things that he loves about life, eg. "You're a county fair in July, Canadian field of wild rye".
The second verse is a continuation of that, possibly remembering her after she passed.
