I think it is about his girlfriend (or someone he understands and cares for) who fails to see her own inner beauty and is on the wrong path. She may have even been unfaithful, and if not to the songwriter, than to herself, seeking fulfillment from sources that could never provide it, as the secret to her own fulfillment lies within her. Thus she is doing some things, and saying some things, that she should not be doing or saying, and getting the results.... a bed of nails, perhaps a double entendre meaning both karma and for being in bed with the wrong person, creating more pain. But all the while, he sees and is wrenched by the conflict between her outward behavior and the inner beauty, wishing that she too could see herself as he does. Its a beautiful and perhaps ultimately compassionate song, about his ability to see through the injury she has done to herself and perhaps him, and his unwavering belief in her inner beauty, despite her behavior.
