Over the course of a long US and Europe tour, Blake had to have throat surgery in London. He was coughing up blood, and had to have polyps removed.

So this song is about this experience.

"A little voice that's not quite your own/Count backwards from ten./Yellow jelly shot hard in vein." --
When you're put under, the anesthesiologist tells you to start counting backward. You're usually asleep by seven or six.

"Wathc the lights go from head to toe. / Doped up and coasting down the hall."--
Being carted after the procedure to the recovery room.

"Now I'm talking through my pen. / Do you read me? Am I bleeding? Am I bleeding again?"--
Can't talk to people until your throat heals. So you have to communicate by writing what you want to say.

"Wake up screaming so far from home. / Incontinent off continent. / The tears are warm. The body's numb." --
The pain medication has worn off, and he's awake -- scared being so far from home in a hospital.

"Get your coat, your ride is here."
You're recovered "enough". Now get the hell out, so another patient can have your bed!

To me, this song goes hand-in-hand with "Tour Song" -- a first-person experience about playing live. Where they've given it their all, squeezed every bit of themselves that they could from these songs that night, and it's still not good enough for the audience.

Song's meaning is pretty straight forward.
Being a band that's trying to make their mark SUCKS. Tour van breaking down, driving for hundreds of miles, eating bad food, unbathed, and tired.
And when you finally get to the gig, you find yourself playing to a handful of people who couldn't care to see you play anyway.
And then, you start playing, and that washes away, and you kinda remember why you wanted to do this in the first place: it's to play the music. And you lose yourself in it.
And maybe, just maybe, you connect with just one or two individuals in the crowd -- like the couple in the back who don't have the hardened cynicism the rest of the crowd has.

I think this song is a dig at Gilman Street, the Oakland, CA all-ages club where bands like Green Day and Jawbreaker played frequently. Alternately/simultaneously, I think it's also a shot at San Franciso-based fanzine Maximum Rock N Roll, who has been criticized in the past for being judges of who/what is considered punk. I think that Blake felt somewhat ostracized or judged harshly. by both this club attendees and the zine because Jawbreaker wasn't punk enough. That there's a punk checklist, and if you don't respond correct to five of the seven criteria, you're not punk.

Instead, Blake is saying, who the hell is punk anyway? It's probably his friend Boxcar who couldn't give a rat's ass about the scene or monikers anyway.

In all, Blake couldn't care less about "punk" if it means barriers put up between people.
