I believe the diamonds he references are ‘perfection’ (perfect person/mate) but, he loses interest. Perfection does not hold his attention as he gets close “they start to fade”.
The person he is singing to clearly expected more from his attentions. He regrets failing them, but in the end it was their expectations of him. Never his intent. He is sorry for the pain/upset caused, but he is the way he is. Perhaps even wishing he was different.

Though initially this does appear to be about someone who is dying, I believe this is about someone battling a [bad] addiction (and most likely succumbing to it - e.g., heroin), and is reaching out.

The lines "Slide up next to me,... I will take the blame" people may be reluctant to approach someone who shows the physical signs of addiction; taking the blame could be for getting addicted. I think this is further supported by "I will take the blame" and "this is not me", "I'm better than this", "Enough, I don't think there's such a thing". People battling addictions will do things they never would otherwise.

"Having caught a sickness in my bones" - addiction and attempting to withdrawal can be painful.
"How it pains to leave you here.... on your own" - when someone is in the throes of addiction everything falls to the side and satisfying the addiction becomes the primary focus, and everything else becomes secondary.

The "just don't let me go", "help me see myself ...", "it's hell", "but no one hears before I disappear" - also lean towards the person is trying to fight/overcome the addiction, but is loosing the battle (loosing themselves and becoming consumed).

Either way, this is a beautiful song.

To me it sounds like Bono is saying 'Baby every dog on the street....' not 'maybe every dog on the street'

General thoughts, you get mired into your habits, what you think is the reason/reality maybe isn't the stark truth, and we chase futility or self-sabotage.

So far this is my favorite song from Songs of Innocence.
