I personally think that the overall theme of this song is miscommunication.
Someone is being a bit two-faced, and says one thing one day but then the next says something and the other person has no idea what the relationship is about and so what they're saying is your actions are what is going to show me where your heart really is. And because of this lying or whatever it is, this double-conveyance, this person says they're "heart's not in it". This person is in love with the other but the other is not telling them properly, what their feelings are:

Then the other starts to become involved in the relationship and wants to dig deeper but they're still sending mixed messages.

Well, being someone who recently thought she had lost her best friend whom she cannot live without, I'll say this:

I think this song is COULD be (and I stress the 'could') about someone who had a falling out, a fight I guess with someone very, very important to them because they did something extremely stupid, maybe they were jealous of someone else who was close to that close friend, hence:

But don't answer that

(^ Those are the actual lyrics, I've been listening to the song on repeat and I'm pretty sure those are the lyrics.)
So this person has lost this relationship, and I think 'But don't break your back' is like saying "Don't give in" because if we want to think about it, if you broke your back you would be paralysed, and I feel as if this is saying you would be not literally paralysed in body, but in soul and mind if you did give in. You'd feel like all is lost and there's nothing left for the world to offer, nothing is worth anything anymore. But yes, that's that part, then the "If you ever hear this, but don't answer that", I believe that the "hear" part is talking about the things you heard about the other two when they were together which made you jealous, and then the "But don't answer that" is like the person is looking back and saying "You shouldn't have answered the questions that were asked of you concerning the relationship and your take on it." The person shouldn't have questioned that relationship and shouldn't have divulged the jealousy so that the close friend who was drifting away (he/she is drifting away indeed I forgot to mention you know now) because it just made them irritated that the friend didn't agree with the relationship.

Bulletproof vest: What you wear to protect yourself from further hurt.
Windows all closed: Trying to block out all the hurt you're taking from the world and that close someone.
I'll be doing my best: Trying to recover from the hurt.
I'll see you soon: Saying that sooner or later I'll be stronger to come back and talk to you about what happened and maybe it'll be fixed.
In a telescope lens: When you were jealous you were overanalysing everything the other two were doing without you, telescope lens magnifies things and lets you see closer and so everything they did was a bigger deal than it probably really was but just because it was without you you felt left out.
And when all you want is friends: All you want is your close friend and your friends to support you but they don't understand.
I'll see you soon: Same as before.

They came for you: When they found out about your jealousy they came and found you so they could talk.
They came snapping at your heels: They kept questioning you, your friend finally said he/she would rather the other person and left you.
Etc. etc. and the rest is the same.

Most of this is actually hypothetical, my friend and I are perfectly fine now, but I do have another friend who went through this and she is going through shit. This song really strikes a note with me. Just everything about it. How it can mean everything in so few words.

Unfortunately, I think this song is about rape.

This girl was madly in love with this man and trusted him dearly and never even considered that he could be a bad person.

But he does rape and kill her and her heart is broken badly as she is dying because he was the reason for her life and he's the one who has taken it.

He's the only one who knows she's dying because he's the felon, 'tell me when you hear my silence' when she's finally silenced and dead, 'there's a possibility I wouldn't know', she's in too much pain psychologically and physically to even think about staying alive or crying out, plus when she dies obviously she won't know because she's dead (clearly).

When he leaves her (after she's been raped), she'll fall obviously I mean she's dead + 'by blood and by me' is referring to her physical and psychological pain.

Just think about it it makes clear sense.
