Here's what I think the second verse means (If I go by what you've been saying):
"We drank the great lakes, Like cold lemonade
And both got stomach aches, sprawl out in the shade"
Drinking the great lakes represents all the medicine he was given to try the wake him up. However, the medicine didn't work as he got a "stomach ache" The shade is just to rhyme...

"Swore to death, you held you're breath"
The doctor is worried, and doesn't want him to die, and so is holding his breath, praying he won't die.

"You made my frown turn upside down"
The doctor, in being his friend, helped him die happy, instead of with a frown.

"Now my worries are gone"
He is now able to pass freely into the afterlife, without worrying about anything.

Yeah, I agree with you, it is kind of like that, until their relationship blows up and he is a depressed wreck...
Let's hope that's not what this song is about =P
