I disagree about the patient begging for help. She seems to resist help more than ask for it. This is made clear on Sylvia, as she hides under the covers and refuses to take her head out of the oven. She does nothing to fight death.

Sounds like he is reminiscing on a past relationship. It took place during what he deems his "wonder years." He and the girl didn't stay together and he often thinks back on the relationship. He's not terribly heartbroken, just sounds like he wouldn't mind if things had worked out. |
