| This song is not necessarily only about tourette's: It is using the outbursts associated with the condition to convey the feeling of loss of control associated with panic attacks. And the "cold heart, mean heart" bit is about how when in that state of mind you feel as if no-one understands, and absolutely no-one can give you relief.
