This reminds me of a book I had to read at school. It's called Effi Briest by Thedor Fontane where a young woman falls in love with an interesting man (protagonist) though she is already married to another man. When he gets it, he wants to duel that rival and kills him at the beach. Hell of a boring book, but matches perfectly. |

I think he and his girlfriend broke up and she treated/ is treating him like crap: "Poured salt on my wounds when my stitches tore".
Thats why he says "And now I'm giving it back cause payback's a whore", but to do so he needs the lobotomy, a surgery which may change personality, because he might still have feelings for her I guess. And his new personality "would make it easier to throw it all away".
