I don't think this is about suicide, I think its his love affair with a girl who has "issues", like her "carpetbaggers", i.e. people who follow her around because she's close with Bruce. And the reference to Bellevue is because she has mental issues. Bellevue was a famous mental hospital on the East Side of Manhattan. Her ability to "laugh and cry in a single sound" leads me to believe she was bipolar, maybe? And the reference to her "strength is devastating" leads me to believe she survived despite having this serious illness.

I think this song reflects the highs and lows of manic depression, which some think Elliott suffered from. During the manic phase, you stay up all night and feel "pretty". Then, when the depression phase hits, you feel "ugly". |
