Here is my take on this song:

The drunk driver kills the girl who is wearing her seartbelt.

The song is written from the point of view of someone who witnessed the crash (I think ghost must mean - he wasnt there; he didnt do anything), saw the girl was stuck inside, thought about helping but paniced and it was too late, he is now haunted by the image of the poor girl and feels immense guilt for not helping soon enough (or at all).

The ghost will remain bit refers to being haunted by the guilt.

The keep my casket closed line might mean it will be something he will take to his grave and will always feel responsible for.

oh yeah one last thing im not sure who/what his enemies of the last five years are in reference to.

It could be either parents/people who disapproved of the relationship, myself himself, or literally his enemies gloating in the fact he will now die alone.

Here is my take on the song:

Basically its a story like romeo and juliet..both considered take their own lifes to be together BUT Romeo cannot go through with it, bottles it, and ultimately ends up just killing her as a mistake and not himself (russian roulette is the method they used).

This is after the event - people are obviously angry he killed her.

This bit might mean that he tried to cover up the 'accident' by faking a car crash? - Make sure you're still alive, just in time to kill you."

Thats my take anyway, i think the title - Not Good Enough For Truth In Cliche; hints that by not going through and killing himself as well he is "not good enough for truth in cliche (aka joint suicide is a cliche way to die to be together forever)
