To me, "neurosurgeons scream for more at paranoia's poison door" kinda says that on the one hand the people are becoming more paranoid and suspicious, locking themselves away and refusing medical help. But also that the neurosurgeons [who operate on brains] are desperate to find new patients. So that's either a roundabout way of depicting a broken medical system/economy - or just plain that the people are all 'brainless'. |

It's about having OCD. David has mentioned in interviews that he used to have obsessive-compulsive tendencies, and that 'the worm' is that constant, nagging need to follow a compulsion. He is 'not in control' of his actions, because the worm is always around. His parents think he's possessed, but he just has obsessions. |
