The girl is pregnant, having a baby could be worthwhile but it could mess things up as well. She has to decide
with grace, and not be design. (I think he's implying that having an abortion would be best)

"She smokes like guns to the world

But wish you died anyway"

Doesn't sound like a very suitable mother.

You're deciding on them all"

She's suicidal, but she's made the decision to stay alive. He's trying to convince her that she's doing ok.

She's a state, but he needs her. He wants to try to live the way she wants to, but it doesn't work for him either, but he can't cope without her.
