I like this one a lot and I see that so many other do too. My take on the lines:
"Son of Sam, son of a doctor's touch, a nurse's love
Acting under orders from above" comes from the John Irving book The Cider House Rules. I mean the character is named by a nurse in an orphanage where he stays on with a Dr., and couldn't he be seen as being a child of the state (Son of Sam)? I haven't read all the book and the connection is a bit far out but it's what I thought of when hearing the songs a few times.
