I agree with the whole writer's block point of view.
But in AP magazine, Anthony said that the lyrics:

"Lock myself up in a room without a window
Just to see if it was any easier to breathe"

were about him being locked up in a hospital after not
eating for a couple days from being so stressed and
whatnot.
Either way this song is one of my favorites on the album.

The cocaine suggestion makes sense when he says,

"You'll tell everyone that it's so much fun.

The 'happiness' is the cocaine.

I have listened to the song again and again and,
believe me, it has to be one of the greatest songs
I have ever heard. From the lyrics to the composing
of the music, what else can I say? The song is
awesome.
I have read the different interpretations/theories of
this song. To me this song is simple:
A person is looking over their life. Should I say their
"rise and fall"? At one point they had everything a
young person could want; Power, money, friends,
popularity and now time has passed. They have
settled into their life. Personally, I feel that possibly
the person is on their deathbed. "Feel the fear in my enemies' eyes" Nobody wanted to go up against them. "And I discovered that my castles stand
upon pillars of salt and pillars of sand" Nothing lasts
forever. "One minute I held the key next the walls were closed on me" They were accepted by all, then disowned for someone new. "Jerusalem bells were ringing, Roman calvary/Catholic choir are singing". Either way the song is referring to death.
"Never an honest word" could refer to bad business,
dealings, or telling someone 'I love you' to get what
they want. To me the song is simple. "I used to rule
the world" (it was all about me) "Now I sleep alone",
(The fame and fortune are all gone, it's just me.)
"I know Saint Peter won't call my name". Because of
the past wrongs, they won't go to Heaven. The person feels their not worthy. "Revolutionaries wait
for my head on a silver plate". The people that have
been wronged by this person, maybe in love or
business dealings want their revenge. "My missionaries in a foreign field". The friends they once
had that now are nowhere to be found. "Who would
ever want to be king?" They see the mistakes they've made and they can't believe they ever wanted to be #1. Once popularity goes to your head,
there's no going back.
"But that was when I ruled the world." Back when I
knew it all.
