The second half of the song may be refering to a girl he is in love with and would do anything for but cannot work up the courage to tell her his feelings:

"And leave it to me to not speak
when I pass you on the street."

Any ideas to the significance of the title?

I've always thought of thought of the beginning of the song as a break-up of a relationship.

Walk to your house on my lunch break
Here it comes
"Come inside and pour a drink"
You'll probably tell me everything you think
Here it comes
The place and the time that we knew that things could go wrong

I love the line, "Now we're drowning in birthday cakes"
I think it is a reference to mortality and getting older and not the burden of a child.

Okkervil River seems to be really into this song. Not only did they recently cover it, but it is also referenced in at least two of their other songs ('You Can't Hold the Hand of a Rock and Roll Man' and 'Love to a Monster'.)
