Elephantiasis is what you're referring to, and Joseph Merrick actually had proteus syndrome, which is a different disease entirely. Elephantiasis is actually caused by a parasite, whereas proteus syndrome is a genetic anomaly.

I believe the initial lyrics are "when the roll is called up yonder" In interviews they have said that they grabbed a few lines from an old gospel song, and the lyrics work well there.

Dogs are actually wolves to put it more correctly, and yes the mocking mans best friend is a reference to dogs, but at the same time on a different layer, it means that the wolf is something familiar but malicious.

Definately about a guy who has either gone through a divorce or breakup and is watching his ex replace him with another guy. He sees the lantern burning in "our home" not your home, ours. He comments that its been a year and whatever the new guy has that he didnt still has her screwing him. Theres a little bit of a psychotic twinge too with the second verse "You should have closed your windows and got another dog. You should have chained up all the doors and switched up all the locks." Basically means something is gunna get you, whether it be time (the pendulum) or the songwriter possibly. any way its an incredible song.
