He may have or may have not been taking about heroin. But to me this song feels like a fierce battle with addiction, whether it be dope or alcohol or crack. addiction is addiction in my book. and not a single song is about drugs if you dont want them to be methinks. i love you, elliott.

also one more thing, it is very likely he wrote this while under the influence of k. i was very able to write my thoughts while with k. in fact i felt the need to since i could not remember anything about the trip otherwise.

from very recent hard experiance's with dissocitives (ketamine, dxm, etc.) this song is very meaningfull for me. i listened to this song yesterday, before coming to this site, and came to my own conclusion of this being about ketamine. it is very hard to explain the experiance of dissiosiatives without sounding crazy but i will try.
the first verse is about picking up ketamine "down on the dock". boy in blue meaning a boy who is sad. aka elliott. sadness wont stay away from him.

the drug takes you to similar thinking everytime you take it. it is a very different kind of thinking. hence pretty mary k taking him back to town. when on this drug you feel a sort of pounding noise in your body. hence the "where the water pounds". when with mary k, you tend to cry from the drugs intense battering on your mind and body. the drug hurts mentally and physically. hence Up against the wall crying black and blue". this effect usally keeps users at bay afterwards. which is true in my case. i have no desire to do this again. when under the influence of mary k it almost seems like god is taking to you with very clear and deep thoughts that can be very pretty. hence "Pretty words that you whispered". it can be very confusing and overwhelming also. the cognitive memory is very mixed up while with k. some things make sense and some things dont. it feels like your not listening close enough when it doesn't. hence

when with k it feels like you are actually with someone or something that is talking to you or sending you these thoughts and messages. and you can be sure of its reality at one moment but then another you realize the reality that you are just on a drug. hence

But I know it's just a reflection of the moon
A big fake resembling you"
The next line is about aquiring and ingesting this god awful tasteing drug. "I'm going to go down in the water
Fill my mouth up full of sand" Then waiting for the effects to come on, which is up towards 2 hours, depending. and dead imagination being normal thinking compared to k thinking. hence
"I'll be waiting still impatient
With my dead imagination while you're with some other man" the same goes for the last verse.

i can see how it would be easy to think he is talking about a prostitute. and maybe he is. who am i to say he isnt. it just doesnt make sense to me. considering the song is called "PRETTY MARY K". K for Ketamine. I would also like to say that i do not recommend doing this drug at all. I have permanent cognitive thinking damage from this. Plus it is scariest thing i have ever had to endure.... I LOVE ELLIOTT.

I think he is talking about that general and natural sense of impedeing doom. His own nervousness and anxiety. then the song goes into how drugs seem to temporarily relieve it for him. because once its done, it comes back. it seems to me he recognizes the vicsous cycle. The part "My body's running....", feels like utter hopelessness. in the last verse i think he is talking about trying to find the answer but also at the same time having the feeling that the search is futile. and being scared that he might actually find it. i dont completely get this though. "Like a drug in the water"= melting spoon. the riot will soon come following mixture of drug with water in the melting spoon. "a punch in the stomach......." abuse from step father causing this. fact: 90% of drug addicts were abused/neglected in some way by the same sex parent. This song, among others, has impacted my life profoundly. thank you elliott.
