the repitition in this song does refer to the title, its also saying that change has to happen you cant avoid it, the diesease part is ambiguous because its a bit random but maybe its suggesting that the person you love maybe doesnt love you back like a 'disease' and they dont need it becuase its painful. etc |

to me, this song is about love, heartache and starting again. i agree the the other comments it is ironic about the blind fold and 'how can we wear our smiles with our mouths wide shut' they really are beautiful lyrics and every line has some sort of meaning. and the irony of the title sums up the song meaning, with hospitals intending to cure people (love curing people making them happy) however there are some who smoke and cant benefit from the love that has been broken or something.
