To me, this songs about continuing to do drugs in hopes that it's gonna help, but it really doesn't. "I'm not surprised at all and really, why should I be? See nothing wrong" represents to me the feeling of even though he knows he should quit the drugs, he still does them and after a while doesn't even think about the fact that he continues to use and justifies it anyway, even though deep down he know that it doesn't help. Such a true song that hits me every time I hear it.
