I think it is about a guy who is dealing with drugs or someone who has a totally different view on the world than the rest of society. Or maybe him being on drugs makes him see society in that light.

He’s a stranger to some
And a vision to none

Could indicate that "in theory" no one gives a shit about druggies, and he can't "get enough of the one" or "the one" representing the drug he is taking

The drug addiction in itself, He knows the craving is coming, it calling him yet he fears it because he KNOWS how it is without it... How his perspective of the world is without the drug....

The "map of the world part" could be on this "drug trip" this is how he sees the "true world" but "on his face" is the drug wear done onto him

through the blood he can look

Could be talkin about, how many relationships he has lost through his drug abuse... "from the count to the one" The drug itself... How many lives he's seen this drug take, not nessicarily physically but mentally... That "the mountain" he sits on is his life that could of been "gold" pure, but drugs got in the way so it is "shit and or sin" And he'll deside when he's done hiding behind this drug "playing innocence" blaming his problems on the drug or hiding his problems behinde the drug usage...

He wants to quit but fears if he does he won't be able to deal with life, or that he has to face reality for all he has lost already to this habit. It's what he is use to and what he has lifed to and for and what he will most likley die to....

I know it is out there but hey I am HUGE on metaphores!
