I think it's about a friend trying to help a friend deal with extreme depression.
"Throw away the radio, suitcase, keeps you awake"
If you're really depressed, you listen to sad songs. You wanna get away from wherever you are, so much so that you can't relax, can't sleep. The friend is telling him/her to stop listening to these songs, stop thinking about escaping.

"You need to understand - there's nothing strange about this. You need to know your friends... you need to know that. I'll be waving my hand, watching you drown, watching you scream. Quiet or loud."
This person's friend is telling him/her you're not the only one in the world going through this; you're not alone; you don't need to feel alienated. Even in this state, you're accepted by your friends. You need to know you have people who care about you. I'm waving my hand for you to see I'm here, watching you suffer, both when it's obvious to anyone, and when it's quiet, subtle, but still noticeable to me (but I still can't really help until you let me in).

"And maybe you should sleep.
And maybe you just need a friend.
As clumsy as you've been, there's no one laughing.
You will be safe in here. You will be safe in here."
This person is suggesting to him/her, that maybe you should rest, should at least for awhile forget about what you're going thru. Or maybe you just need someone to be there, to support you, to understand what you're going through. Even though you haven't been yourself, you've been awkward and clumsy in your demeanor, your attitude, your interactions with everyone, nobody is making fun of you or thinking less of you now. With your friends, and in your friends' opinions of you, you'll be safe. Whatever you share with them, however you are around them, you don't need to worry, you're safe in them.

"Throw away this very old shoelace that tripped you again. Try to shrug it off, shrug it off, shrug it off - it's only skin now.You need to understand - there's nothing fake about this. You need to let me in. Watching you and... I'll be waving my hand, watching you drown, watching you scream.
No one's around."
This person's telling him/her to stop dwelling on whatever's causing this pain, both the material stuff and the thoughts. Let it go, whatever's hurting you doesn't really matter. Realize that I'm genuinely here for you. I'm serious and I want to help, but you have to let me past the walls you're putting up. You have to accept my help, let me in on what you're going through. I'm watching you suffer now and I'll still be here watching, waving so that you know I'm here seeing you go through this, even if you don't let me help you. But if you don't let anybody in, no one can help you.
