I think this is about desperation, about a place in your life where you're longing for God. A theology student told me that in the Bible "The Eleventh Hour" meant Redemption.
It's clear throughout the song that He wants God to bring Him back.

"trace the shape of my heart, till it becomes more familiar to your eyes." and "Take the place of my heart, 'til I've become a stranger to my life" speak of how the writer is desperate for God to change his heart once again, and to change it to where GOD wants it.
Desperation and Redemption are the theme, and it's written beautifully
