'without a second opinion' could be a reference to turning off a life supprt machine, in order for passive euthanasia to happen two doctors must agree on it. this would fit with the repition of 'turning off' and references to a critically ill friend 'we are coming to the capital' i agree with vtmatt that this is most likely to do with government, but could the capitol be a metaphor for the dying person maybe? |

'needle' and 'willing arm' 'quitting alone' all hints to an addiction.. of course this is probably metaphorical and i agree.. its about making love in a car |
