At first, the character seems to be pondering mortality, the existence of God and eternal life.
I think the line "Looking back, seems that everyone in my life will be waiting for me," is saying that this person has lost a lot of loved ones and seems to believe that when they die there will be a reunion.
The line, "Have you found your way home?" is possibly asking if someone has made it to the afterlife, to heaven, but I'm not completely certain about that.
The last part that Simon sings with "All the way down" being repeated in the background to me could be about nothing else but the character's impending suicide. First there's the R.E.M. reference.
