I think this song is about the paradox about being a young adult and struggling to figure life out. Part of you just wants to get through it as fast as possible and finally be stable ("wake me up when it's all over; when I'm wiser and I'm older"), while the other part is terrified of getting older ("wish that I could stay forever this young").

My reading is that it's about addiction. From the lens that I'm looking through, it's about heroin, but it could be any IV downer. What I get from it is that addiction runs in this person's family ("it's in your blood" and "I can't change the evident" and, most of all, "predisposed to perpetual sickness). Throughout the song, "I can't change the evident" and "can't change the evidence" have some different meanings -- I can't change that this is in my blood; I can't change that I started this drug; I can't change that I'm going to keep doing this until I inevitably die/fade away.

Another verse that resonates to me RE; IV drug addiction is:

"Now that I've dodged your questions so much
I don't possess the strength to answer straight
And no, I'm not afraid
At least not to die
I'm afraid to live and not remember why
Sweet chemical indifference
I can't stop, can't change the evidence"

When someone is addicted to drugs and you are trying to get through to them (in my experience), you're perpetually trying to ask the question that is going to wake them up -- don't you care about your family? Don't you want other things in life? Aren't you afraid to die?

Throughout the entire song, he is addressing someone he loves who had to "make a clean break" and move on with her life.
