I think it's about the struggles of one drug trip gone wrong and of drug dependency in general.

He needs to 'go sit on the bed' and 'lie on the floor' because he's probably wobbly from all the drugs he took. He was also probably brought to a rehab center a coupla times, with the line 'community service was the best job he ever had'/'cleaning up' and had withdrawal symptoms - 'got sick on the floor and your shoes'

On the fourth verse he was taken to the hospital or a doctor (MD). He probably had one too many and had an OD while celebrating. 'She said it's your birthday, are you feeling alright?' Considering Matt Healy writes mostly autobiographical stuff, the 'she' could be his mum. His brother just sat there and watched him through the fiasco, but he wishes he'd ultimately turn out different than he did and make better choices.

His dependence on drugs makes him turn to it to whisk off perversive feelings of youth alienation, to feel validated, affirmed, and happy. The City is where the drugs are. The fix/love is there.

I love this band no matter what they say.
