This song is about the beast, but it is also about his associate: the Antichrist.
The Antichrist is Beelzebub: lord of the flies.
The Beast is, of course, Bael.

Total beast reference. 69 years refers to many things, including the year that he was born, possibly around the release of the Satanic Bible (Dec 1st), Charles Manson's indictment (Dec 3rd), or Altamont Free Concert (Dec 6th)... Meaning he'll turn 40 within a month, the timing may be significant.

This song is about the beast and his post piggy lover. See the backmasking. She apparently helps him get through the suffering of losing his piggies. This may be after a breakup: "First of all, we leave flat" The references to "domestic graveyard" are likely to his piggy(s).

The first three stanzas are sung from her point of view.

The next three are sung from his point of view.

The last stanza is split between them, with the first two lines being her, and the last two lines being him.

refers to what the beast does to her. It is very sad. In a fit of rage he uses black magic to scar her breasts, cutting off her nipples and murder someone she loved very much who was like a father to her.

Refers to her retaliation of infecting him with HIV.

When it all begins, he is the sweetest person he knows:
"I hurt myself today" is what he is talking about when he says
"I will let you down, I will make you hurt"
"My sweetest friend" refers to himself.
As his empire grows and he suffers the repeated tragedies, he degenerates into the creature sung about in "Big Man with a Gun"
The last line in this album is his lament about losing himself:
"if I could start again, a million miles away, I would keep myself, I would find a way"

This song is yet another song about the Beast.
Fall into a black hole sun and you will never reach bottom.
It is the bottomless pit from which the beast ascends.
It is also one of his main symbols.
"Call my name through the cream" and "Won't you come" are references to his use of sex magic to project his image into music in general: "No one sings like you anymore"
There are connections to the backmaskings of the live "Scentless Apprentice", and "Hello" by Babes in Toyland.
The video has even more, including directly suggesting that this song refers to the end of the world: "The end is nigh", and "Faith saves" which refers to the rapture (as does "til you all just disappear".)

Look at the damn backmasking for the live version of this song before you decide what it is about. Cross reference this to "Your scent is still in my place of recovery." This song IS about Cobain's own suicide. When are you people going to realize that music heard by millions repeatedly affects so much causation that it is much much bigger than the musician.

"He" refers to the main character, the beast, as particularly evidenced by the congruency of "he flexed his muscles" with other descriptions in this album. The part about sewing his eyes shut is a reference to all of a.) the new testament biblical references to the blind, b.) the first and later the second fatal head wounds. The virus referred to is a reference to both the beast's eventual infection with HIV and also his brand of Christianity which will be against police and military as is the killing off the swine reference, which is also a slanderous reference to the death of his piggy.

This song is about the physical and emotion pain sustained by the beast when he is shot in the head the first time and loses his piggies. (Probably in an eye causing the vision impairment mentioned.) The phrase "I do not want this." refers both to the escapism which overwhelms him and drives him to hardcore opiate abuse, and also the reaction to the slander that he wanted these things to manifest in his life so that he could become someone important.

This song, even this album, is part of the many signs and wonders of the beast: "You speak religion's message clear." This song speaks of the beast's primary lover, who is referred to as his "piggy". The death of the pig is part of the "bad wolf" meme, which implies that two others will die as well. "All the pigs are all lined up" The beast will apparently be dreading this and aware of it ahead of time as "all of my fears came true". The fact that it was recorded in Manson's house implies a Mansonesque scandal, with their murder being very publicly predicted by messages in songs, and very gruesomely executed. There are connections to backwards masking messages found in the following songs: Frosty the Snowman, Idiots Rule, "Magic Boy, Magic Girl", Killer on the road, She wants to share her magic.
