I'm pretty certain I read somewhere a while back, I think it was an interview with Eric Clapton, that this song is about cocaine. I remember thinking that was bs, it sounds more like a heroine tune, if anything, but it was explained that he's singing from the low point after the comedown; depressed without what he craves, but excited by the idea of its (hopefully) imminent arrival. I can't remember where I read this, so I can't cite it, but I felt I'd contribute the potential interpretation, anyway.
