It appears to me that Josh at some point in time has lived in Norway for a period and experienced the depression that can come with the winters up here. If you are not used to it it can be quite hard on the mid to go through a winter where you only get a couple of hours of grey sunlight every day. You actually get a lack of seratonin and D vitamins from not having sun on your skin for months.
He also says he doesn't understand the language and the doctor just asks him to wait for June,
