This song is very much about consumerism and the notion of "getting high" off of it. The song tries to show the lack of realism people who are rich are act rich have. The $200 bill line is to mention how fake they are much like their appearance, since $200 bills do not exist. The pill they take is the idea that buying your happiness is possible, hence why it can be lonely at the top, they do not simply have monetary problems to worry about. In this case, they create their own problems that are unsolvable.
