Personally, I see the song as Kate is the realist; The one who sees things a bit more 'adultly' than her peers -- Or, rather, the person in mention during the first few lines -- Who sees the World with a bit more infantile point of view. They don't fret over dying tomorrow, or today, but rather, because they have 'time', which I find to be the key point of this song.
Kate's look on life is there is no time -- We could all be gone tomorrow; even in a mere second we could die. She sees this, and then it begins to open more and more worries; more and more vision to the real and immediate dangers of the World that can happen at any given moment. As she says, "Unlike you I am not pretending" -- Unlike the person(s) in question, she isn't allowing herself to be so oblivious to the real concerns and pessimistic views of life. Whereas her peer(s) have 'room to swing their arms', she believes there's 'no time' to do so. The World is a dangerous place -- Whereas her peer(s) see it as a treasure trove of adventure, she believes it to be a dark cave with the growling of an unseen beast.
In other words, I feel the song is Kate's view on life -- There is no time, there is only right now, and it could end in an instant. She cannot be happy and care-free, as she sees things with a more adult view, rather than the infantile and somewhat oblivious view that those around her have.
