This is probably wrong, but I think it's about a guy who likes a girl but then kind of realises she doesn't like him back, and is only using him for, erm... drunken sex, basically. I only came up with that soley based on the puking down the rented tux, and I think that that was the point where he realises that she's just drunk and doing drunk stuff, and might not like him that much, influencing him to say 'I'm not sure if it's love anymore'

"snookered between the back cushion..." Sounds like a dilemma to me again referring to the previous point.
I have been dreaming you've been dreaming about me" is being said from the female's perspective, whereas the rest of the song is from the male's.
He likes a girl a lot. The girl likes him, too, but only for drunken sex. “This is the crux.” She is seeing other people as well. “Remember what your heart is for” means that he does not want to have sex with her if it not going to be for love, because his heart will be broken if he does. So he does not have sex with her, at first. He worries about being killed by her other boyfriend. “It’s a good night for a fist fight.” This death would be "by your hand," because she would be responsible since she led the protagonist along and caused this. He fantasizes that she is dreaming of a long-term relationship with him, even though he knows deep down that she isn't. He ends up having sex with her on a pool table “I lay you down atop the baize.” This time he didn’t remember what his heart was for and soon his heart is broken. The last “by your hand” refers to his own death, a suicide caused by her inability to return the love he has for her.
I think all in all the song is about thinking of a person while masturbating. "I've been dreaming you've been dreaming about me" sounds like a fantasy to me. Plus the whole thing of being blue-balled by a girl who is too drunk, and so he jerks off while thinking of her as being graceful, with thighs like a stallion (which is a line I love). I think this is almost proven with the lines "I'm not sure if it's love anymore, but I've been thinking of you fondly for sure" he's thinking about her while jerking off, knowing he only likes her for her body I.e, not love.
