"Fast car" is kind of a continuation of Bruce Springsteen's "Born to Run." It has all the clawing your way to a better life, but in this case the protagonist never makes it with her love; in fact she is dragged back down by him.
There is still an amazing amount of hope and will in the lyrics; and the lyrics themselve rank and easy five. If only music was stronger it would be one of those great radio songs that you hear once a week 20 years after it was released. The imagery is almost tear-jerking ("City lights lay out before us", "Speeds so fast felt like I was drunk"), and the idea of starting from nothing and just driving and working and denigrating yourself for a chance at being just above poverty, then losing in the end is just painful and inspiring at the same time.

I think your first comment is closer to being accurate. The singer/song writers state "Millions of eyes can see, yet why am i so blind!? When the someone else is me, its unkind its unkind". I believe hes referring to the girl toying with him and using him. He wants something deeper with her, thats why he allows himself to be as a puppet (even though for her fun and games) as long as it makes her happy. But he knows deep down that she doesnt really want to be serious with him and thats what makes him.

I actually think this is the last song in the story. So fist Caleb, then end of this chapter, don't say a word and finally juliet.
During "don't say a word" he tries to kill her (maybe he stabbed her or poisoned her) after which he commits suicide (as he wants to die togeter with her). "Juliet" then describes the stalkers death ("These are my final lines, I've used all my nine lives").
I'm currently wondering whether the girl survives: some things seem to point to her surviving ("But why do you smile?", "You're closing my eyes..." which seems to indicate she isn't dead whereas the staler is) while others point to her death ("My life is over, cannot feel your heart anymore")
I don't see how the "You had a change of heart, Now we're all getting hurt..." would fit into this though...
Cloudslayer, to me, "You had a change of heart, Now we're all getting hurt..." maybe is a reminder. If that Juliet was still with him, they would still be living together, happy. But since she had a change of heart and decided that she didn't love him anymore, Caleb felt like he was betrayed since she walked away smiling. Now the stalker says (or so it seems to me) that because of her decision, they will all suffer. Him because he lost everything he trusted and, her because he can't let her live happy without him. Might be wrong, but I like it this way ^^
