The narrator of this song is taken aback by someone. They didn't expect this person to show up in their life (blow by blow I didn't see it coming), but he or she feels it in his or her bones that the other person is there to stay. It is completely based on intuition and not intellect or fact. (I feel it in my bones.)
The narrator also feels that the other person feels the same. (You're slow to letting me go.)
Apparently, they are on a break right now. Maybe the connection was a bit scary as it was unexpected, especially for the other person. (Run away. Rushes out. You always run away.)

I think this is a song about losing someone who you still love.
In the beginning of the song she is clearly saying how she is in love with someone and will always have feelings for this person.
"What rushes into my heart and my skull I can’t control" She can't help who she loves no matter what happens. She has no control over her heart and can't get this person out of her mind.
"You're slowly letting me go, I know this feeling oh so" Is her realization that she is losing this person. She seen it before and even thou she tries to hide it, she knows deep down this person is leaving her.
"left hook" and "You got dead aim" is the shock of her loved one finally leaving. She didn't see it coming, it was unexpected.
"tell me you are here to stay" is basically her saying "please tell me I'm wrong and you will always be here"
"I take my heart out of my chest I just don't need it anymore, take my head out of the game I just don't need it anymore" She is so hurt and now has a broken heart (which is why she don't need it, you don't need something broken). Head out of the game is her way of saying she can't handle loving and losing someone again.
That's my idea of what this song means. It's still a great song even if it's kinda sad.
I love when Sara says "you are here to stay" and "you always run away" its amazing :)
It's talking about all the things that pop into your head that you can't help but think.
The emotions and the feeling of "Well, I'm done trying now".
