As for meaning, to me it's about starting a new relationship with someone. Chances are it's not going to last, no matter how it feels at the beginning. I think it could be about whether or not a relationship is worth having when you're unsure of its outcome, but he seems to be saying that sometimes it's worth taking the chance.
Maybe a bit cheesy, but I like the song!
btw, for WWE viewers, this was HBK's HOF theme song.
To me, just that makes the song even more beautiful. He's a musical genius and I love his songs. I apply it to love as well and I think that's what he is mainly singing about but it also boils down to life in general. His quote explains it all.

Originally, I thought it's just about life. There are so many things that are left to chance...
Big chances like, there's a chance you won't live to be 30; and then the small chances, like, There is a chance I'll, I dunno, suddenly decide to wear blue tomorrow.

But the more I read the lyrics, and listen to the (awesome) song, The more I see the relationship thing...

In my opinion is that him and this girl who he loves deeply had a horrible breakup. Throughout the whole song its saying that he has chances to talk to other girls, but he can't get that one girl out of his mind.
He tries to tell this girl how he feels in this song.
but, this is what i think it means. i mean, i could be completely wrong haha, but it makes sense to me.

He is asking himself to be sensible. He knows that when it comes down to it there’s no way he could be one of the lucky ones. (Chances are, when said and done, who'll be the lucky ones…) And even if she says that he could be her answer he knows nothing will last forever. No matter how they feel about each other right now (“No matter how it feels today”).

And then he says that they’ll probably find a new “equation”, which means things will probably change. He doesn't dare take a chance, because he’s afraid of what'll happen and therefore the chances are slipping away from him.

Still he can’t help but fear that he might lose her someday (“Afraid to love beyond what I can lose”), because he’s a realistic man and he knows that these things often happen. He knows that it doesn’t always last and that people go their separate ways. But he’s still willing “to see them through” which means he want it to last and he’ll fight for it.

Again he’s saying that they’ll probably find two separate destinations, which means they’ll have to leave each other. But there're still "possibilities", there's the possibility that it might work out.

It seem s that what he is trying to say is that there is always chance about everything and that things can go right or wrong. mikerobe007 seems right about the fact of starting a new relationship.
What I got from this one was that you're either in a relationship that you feel is nearing the end or you're in a relationship and being realistic that the chances are it will not last. Look at the divorce rate for the U.S., the numbers are nearly 50-50!
In some of his lyrics - like the line "no act of god could pull my away from you" -- that makes me think the relationship is not over or at worst they are taking some type of break. The line "and though I see us through" -- he means I see us through this, not through as in done.
The whole two destinations thing in my opinion runs along with the fact that we always see the life we're living and there's always an option to do something else. That's always that second destination. And in the end, life is always about chances. "Chances are only what we make them."
